Remember that refinery shutdown in Beaumont back in '08? The new safety regs after that changed how we do every single confined space entry now.

Man, that whole era was wild. We had a similar wake-up call at a chemical plant upstate. After an incident, they made us start using these huge tripod setups with winches for every single tank entry, even the small ones. The paperwork alone took an hour before you could even put a boot inside. It felt like overkill at first, but you can't argue with the results.

The tripod and winch is the way to go... we had to start using a second attendant outside with a dedicated comms line too. It adds time, but having that extra set of eyes and a clear way to call for help is non-negotiable now.
